Participant Agreement, Assumption of Risk, and Release of Liability

Welcome. Please read this before you practice with us. It is a legal agreement that affects your rights. By signing, you agree to everything below for yourself and for anyone you are signing on behalf of.

In this agreement, "the Studio" means Flow Together Studio, Flow Together LLC, located at 2720 Frankfort Ave, Louisville, Kentucky, together with its owners, teachers, staff, volunteers, independent contractors, and landlords. "Activities" means anything you take part in at the Studio or at a Studio-hosted event elsewhere — including yoga, qigong, meditation, breathwork, chant and song circles, workshops and trainings, community gatherings, use of studio props and equipment, the lending library, the creative corner, and any food or beverages offered.

1. Participation is my choice

I am taking part in the Activities voluntarily. No one is requiring me to participate, and I may stop, rest, modify, or leave at any time. I understand I am always invited to move in the ways that feel best in my body, and that I am the best judge of what that is.

2. I understand the risks

Physical movement and breath practices carry real risk. These risks include muscle strain, sprains, fractures, dizziness or fainting, heat or breath-related discomfort, aggravation of an existing condition, and in rare cases serious injury, permanent disability, or death. Risk may come from my own actions or inattention, the actions of other participants, the condition of the space, floors, props, or equipment, or the ordinary negligence of the Studio.

I also understand that meditation, breathwork, and chanting can bring up strong physical or emotional responses, and that I am responsible for choosing how far to take any practice.

Knowing this, I knowingly and voluntarily assume all risks connected with the Activities, both known and unknown.

3. I am responsible for my own body

  • I am in good enough physical condition to participate, and I have been advised to consult a physician before beginning any new physical practice.

  • I will tell the teacher before class about injuries, recent surgery, pregnancy, or anything else that affects my practice, and I will keep them updated if that changes.

  • Hands-on assists are offered only with my permission, and I may decline or withdraw that permission at any time, for any reason.

  • Teachers and staff are not medical providers. Nothing offered at the Studio is medical advice, diagnosis, or treatment, and no teacher has evaluated my fitness to participate.

  • If I am injured or feel unwell, I will stop and tell a teacher or staff member.

4. Release and waiver of liability

I RELEASE, WAIVE, AND DISCHARGE the Studio from any and all claims, demands, causes of action, damages, and liability for personal injury, illness, death, or property loss arising out of or connected to my participation in the Activities or my presence at the Studio, INCLUDING CLAIMS CAUSED BY THE ORDINARY NEGLIGENCE OF THE STUDIO, ITS OWNERS, TEACHERS, STAFF, VOLUNTEERS, OR CONTRACTORS. I agree not to bring a lawsuit or claim against the Studio for anything covered by this release.

This release does not apply to gross negligence or to willful or wanton misconduct, and it does not waive any right that cannot be waived under Kentucky law.

5. Belongings, borrowed items, artwork, and refreshments

  • The Studio is not responsible for personal belongings that are lost, stolen, or damaged.

  • I borrow items from the lending library at my own responsibility and will return them in reasonable condition.

  • Work in the creative corner is offered by independent local artists. Purchases are between me and the artist; the Studio is not a party to those sales and makes no warranty about the work.

  • Water and snacks are offered by donation. I am responsible for deciding whether anything offered is right for me and for asking about ingredients before I consume it.

6. Children in the space

Children who are present but not enrolled in an Activity remain the responsibility of the adult who brought them and must stay with that adult at all times.

7. Photo and video

The Studio sometimes photographs or records classes, song circles, and gatherings. I give the Studio permission to use my image, likeness, and voice in its website, social media, newsletters, signage, and other materials, without payment to me. I understand I can opt out at any time by telling a teacher or staff member or emailing lauren@flow-together.com, and that the Studio will stop using new material of me going forward, though materials already printed or shared may not be recallable.

8. Community agreements

I agree to treat the space, the teachers, and the people practicing beside me with care, to follow reasonable requests from teachers and staff, and to honor the community agreements posted in the Studio. The Studio may ask anyone to leave whose behavior is unsafe or disruptive.

9. If I am signing for someone under 18

By signing below as a parent or legal guardian, I confirm that:

  • I am the parent or legal guardian of the minor named below and have authority to sign for them.

  • I have read this agreement, I consent to the minor’s participation, and I accept the risks described above on their behalf.

  • I am responsible for supervising the minor while they are at the Studio unless they are enrolled in an Activity led by a teacher.

  • I release my own claims — including claims for medical expenses or loss of a child’s services — arising out of injury to the minor, including claims caused by the ordinary negligence of the Studio.

  • I understand Kentucky law limits a parent’s ability to release a minor’s own claims, and nothing in this agreement is intended to go further than the law allows.

10. Other terms

This agreement is governed by Kentucky law, and any dispute will be brought in the state or federal courts serving Jefferson County, Kentucky. If any part of this agreement is found unenforceable, the rest stays in effect. This agreement applies to every visit I make to the Studio, now and in the future, until I revoke it in writing. An electronic signature has the same effect as a handwritten one.
